Mr Nicholas Ferran is an orthopaedic surgeon with special expertise in upper limb surgery.  He is currently a Consultant in Trauma and Orthopaedic Surgery and listed on the Specialist Register.  Mr Ferran practices at a number of different hospitals and offices throughout London and is fully registered with the General Medical Council to practice.

Following his MBBS from the University of the West Indies in 2002, Mr Ferran went on to obtain significant post-graduate training and speciality education in the UK.  This includes the aforementioned Trauma and Orthopaedic Surgery speciality, an MS in Orthopaedic Engineering, and prestigious fellowships at The Prince of Wales and Concord Hospital in Sydney, Australia, and Circle on the NHS in Nottingham, UK.   

Shoulder and elbow surgery are a passion for Mr Ferran.  One of his particular surgical interests is in performing shoulder and elbow surgeries under local anaesthesia when possible, with patients awake.  This allows patients to have much shorter post-operative recovery times and avoids the risks of general anaesthesia, which may make surgery otherwise impossible for some patients.  In 2017 Mr Ferran treated a 94-year old woman whom received two new shoulders, something that quite simply wouldn't have been possible had it not been for this approach.

Nick recently operated on my right shoulder to remove a calcium build-up. The decision was taken after having first tried physio as well as a steroid injection. Nick recommended a procedure with a local / regional anaesthetic to be done at the Lister Hospital. The process was very smooth. Nick’s team is clearly used to working closely together. The anaesthetist first numbed my shoulder before using an ultrasound to find and block the nerve running from the neck to the shoulder. This is done lying down with the ultrasound screen in front of the patient in the antechamber to the operating theatre. Nick also used the ultrasound to locate with more precision the calcium build-up. I then walked to the theatre and was made to sit upright, facing a couple of large tv screens. Following additional prep (skin cleaning etc), the camera was inserted to the back of the shoulder to check both joint and ligaments. The images were very clear – and to a large extent fascinating. An additional surgical tool was then inserted to the right of the shoulder joint. It is quite surprising to see it pop out on screen. Nick was then able to locate and remove the calcium, confirming no damage had been done to the ligament. Post-surgery I was brought to the recovery room for about 30 mns and then back to my room. I was discharged a couple of hours later and simply went home by cab armed with painkillers and anti-inflammatories. I was concerned about the level of pain I would experience once the block had worn off but in all fairness the pain was much less than prior to the surgery, to the extent I did not take any pain killers past the first night. For the first 24 hours my shoulder moved very freely and without any discomfort. Thereafter the muscles stiffened but I followed Nick’s recommendation and arranged to see a physio straight away. The muscles stayed stiff for about a week but again the pain was low compared to prior surgery. Now, 3.5 weeks after surgery, I have recovered most of the range of movement. The shoulder muscles need stretching a bit but they are not painful. I have already started doing push-up and building up more resistance in the exercises. The surgery was definitely successful and worth doing. The key holes are already barely noticeable. The reality of the procedure was certainly a lot less daunting than the thought of it! As I said earlier Nick and his team are not only highly professional but clearly used to working closely together thus providing a very calm and efficient environment whilst the patient is not only awake but anxious. The benefit of the local anaesthetic was obvious: short stay in the hospital, immediate discharge, focus on physio immediately rather than post general anaesthetic effects. I was able to use my arm as soon as the block worn off (highly encouraged by Nick to do so!). I also had the benefit of hearing Nick’s comments throughout the procedure so that I knew straight away that ligaments etc were not damaged and that the calcium had been entirely removed. Having what feels like a dead arm is odd but this disappeared after about 12 – 18 hours after surgery. I was concerned I would start experiencing pain at some stage through the surgery but that was not the case. The block simply works! Nick also indicated that they have back-up fast active painkillers if required. The only sensation I experienced was pressure and pulling when the surgerical tools were inserted and taken out but that would not register on any pain scale. I am clearly delighted with the result of the surgery and particularly grateful to Nick and his team. I would highly and genuinely recommend him. I am not particularly squeamish when it comes to needles etc so was not particularly worried about that aspect of the procedure. I was however concerned about experiencing pain during and after the procedure. As I said, however, my concerns were completely unwarranted on both fronts. Good luck if you are about to go into surgery. You are certainly in good hands if you decide to use Nick and his team.

In my first consultation with Mr Ferran he examined my shoulder and movement very gently, mindful of my pain. He explained possible causes & treatment. I went immediately for an x-ray, from which he was able to diagnose a frozen shoulder. After further discussion, he administered a steroid injection. I will see him again in two months for a follow up/check up. Mr Ferran was personable, thorough and very considerate. My entire appointment took 1hr.5m. - very impressive!
