I am a Consultant lower limb Orthopaedic Surgeon with a specialist interest in hip and knee replacement surgery and lower limb trauma, with a particular emphasis on people with osteoporosis. I have developed an expertise with many types of hip and knee replacement designs and am well placed to offer the most appropriate design to my patients. I am actively involved in use of advanced bearings including ceramics, and robotically assisted surgery which I hope will lead to better longterm outcomes when compared with more traditional designs and techniques.
I qualified from University College London Medical School in 1999. A year later, I moved to Wessex and began my specialist orthopaedic training. I was appointed as a Consultant at University Hospital Southampton NHS Trust in 2012. Before taking up this post, I undertook three fellowships. The first was as a senior hip and knee fellow in Sydney with the Specialist Orthopaedic Group, one of the most prestigious joint replacement fellowships offered to visiting surgeons. Two further fellowships followed in Southampton, concentrating on complex hip and knee replacement surgery. I have a doctorate of medicine in the use of stem cells in complex orthopaedic surgery and hold an Honorary Senior Lecturer post at Southampton General Hospital where I work as a full time NHS consultant.
I employ a multidisciplinary approach to hip and knee surgery and believe passionately that the combination of patients taking an active role in the decision making process, together with dedicated nurses, therapists and anaesthetists improve outcomes.
When I'm not at work I can often be found cycling around the beautiful Hampshire countryside or windsurfing from one of our superb South coast beaches.

From our 1st consultation I knew I had the right consultant. He explained everything clearly and answered all the worries and concerns I had. He is a very caring, considerate and understanding consultant. He understood the circle I was in not being able to loose weight due to being immobile - I still can hear him say “how can I sit here and see how much pain you’re in and not help you”. I have had both my knees replaced using the robot. Mr Tilley has totally transformed my life from being permanently in a wheelchair to having my life back. Together with Diane is secretary they make a formidable team.

From having a complex right hip revision in January 2022 to a ‘straight forward’ left hip replacement in September 2022, I was very pleased with the process/ service overall from Simon Tilley Specifically, the initial explanation of the plan regarding the two procedures helped me understand what was involved and although the second procedure was postponed on two occasions, this was outside his control .I was provided with timely information on new appointments by his efficient secretary Diane Gubbins which helped with travel arrangements as I live on the Isle of Wight and need to book ferries.

The treatment and all round service I received from Simon Tilley and his team was superb. Simon Tilley is everything you would want a surgeon to be; clinically excellent, diagnostically accurate, surgically skilled, enthusiastic, decisive, and a really nice man. His concern for my long-term condition was refreshing, his reassurance about the treatment plan was encouraging and my total hip replacement was done quickly and efficiently with no problems of any kind. I cannot recommend him highly enough. I’m 62 years old and have never received such exemplary care from the medical profession and I’m extremely grateful. 7 months on, I wouldn’t even know I’d had a hip replacement. My life has been transformed. I also want to mention Diane Gubbins, whose efficient secretarial support and friendly, helpful approach made the whole process stress-free. Nothing was too much trouble for her, and I felt she really cared about the way my treatment was going. A big thank you is due to her as well. All in all, if anybody is looking for a hip or knee surgeon, I would recommend Simon Tilley without a moment’s hesitation.
